Activating Integrated Home Care (ADI) is simple and quick. At Medihospes, we take care of the entire process, ensuring promptness, efficiency and personalised support for each patient.

 

Who can request the service?

ADI can be activated at the request of various healthcare professionals, or directly by the patient and their family members.

  • General Practitioner  ( Medico di Medicina Generale – MMG)

  • Hospital doctor, in the event of discharge or where continuity of care is required
  • The patient themselves or a family member, to receive tailored support
  • The Social Worker, to ensure assistance for people in situations of vulnerability
A personalised care plan

For each patient, an Individual Care Plan (PAI) is drawn up or, where necessary, an Individual Rehabilitation Plan (PRI), which are essential tools for defining the intervention. For services delivered on behalf of private companies or entities, the plan is prepared directly by Medihospes; for services provided under agreement, it is defined by the competent ASL.

  • Define care and support objectives based on specific healthcare needs

  • Coordinate a multidisciplinary team made up of doctors, nurses, physiotherapists, therapists and healthcare assistants
