Home-based care: present on a daily basis

Caring for people, for us, means being present in their everyday lives, preventing unnecessary hospital admissions and providing appropriate support directly at home. We offer an integrated home care service, designed to respond effectively and promptly to the different needs of those who require medical care and support.

Comprehensive support at home

Through a team of qualified professionals, we provide personalised support that integrates healthcare and social interventions, with the aim of promoting the person’s wellbeing.

  • Personal care support delivered by Oss, home care assistants, psychologists and social workers.

  • Continuous availability, 24 hours a day, seven days a week.

  • Management of non-self-sufficiency through individualised care plans (PAI).

  • Organisation of the home environment to support a better quality of life, particularly in the presence of chronic conditions.

Each intervention is carefully planned, involving family members and key contacts, to ensure appropriate, personalised support that is attentive to people’s real needs.

Mental Health
Understanding and support to promote positive mental health

Every individual is unique, with distinct values, experiences and needs.

For us at Medihospes, recognising and enhancing this uniqueness is essential to promoting positive mental health. Our mission is to provide support that respects and celebrates each person’s diversity, through a professional, empathic approach grounded in attentive listening.

We carry out support, care and rehabilitation activities for people experiencing mental distress, from childhood through to adulthood, including interventions aimed at supporting and relieving families and caregivers, who represent the secondary target of our work.

Our method involves the intensive case management of adolescents and young adults experiencing mental distress, within their own family context.

This is a form of intervention and care delivered in the person’s usual living environment (home, school, leisure settings), outside the traditional hospital setting, with the aim of maintaining regular and frequent contact with the person, monitoring their psychological and clinical condition, and providing effective therapy and rehabilitation.

  • Intensive case management of service users within their family context: this is a form of community-based intervention and care delivered in the person’s living environment, outside the traditional hospital setting, with the aim of maintaining regular and frequent contact with the service user, monitoring their clinical condition and providing effective therapy and rehabilitation.

  • Skills-building and rehabilitation activities aimed at developing or restoring psychophysical and social skills: these activities are designed to engage young people and their families in promoting the skills needed for daily living, caring for physical and psychological health, addressing issues related to living together, school attendance and money management.

  • Support to access activities outside the family context: this provides encouragement to address difficulties related to interpersonal relationships in social settings and associated conditions (pathological shyness, social anxiety disorder, avoidant personality disorder, and the more recently introduced Japanese concept of hikikomori).

  • Rehabilitation therapies including speech and language therapy, neuro-psychomotor therapy, occupational therapy, expressive workshops, cognitive behavioural interventions, vocational and recreational activities, nursing interventions, and educational coaching.

To ensure tailored support, we provide integrated social and healthcare interventions with a strong healthcare component, or intensive educational programmes, through:

La nostra attenzione alle fragilità nasce dall’incontro, dal primo passo verso chi ha più bisogno di supporto. Accogliere è il primo gesto, quello che rende possibile ogni altro cambiamento.

Ogni giorno ci impegniamo per offrire una risposta concreta a chi è più vulnerabile, in particolare ai minori a rischio di abbandono o emarginazione sociale. Per loro, abbiamo creato le nostre Case Famiglia, un rifugio sicuro dove bambini e ragazzi sono accolti, ascoltati, protetti e valorizzati.

Non ci limitiamo a fornire un luogo sicuro, ma li accompagniamo in ogni fase della crescita, sostenendoli nello sviluppo emotivo, sociale e psicologico.

Crediamo che ogni giovane abbia il diritto di costruire il proprio futuro.   • Ascolto e Supporto Psicologico: Siamo qui per ascoltare senza giudicare, offrendo un supporto psicologico che aiuti a elaborare il trauma e a riscoprire la forza di affrontare il futuro.
  • Consulenza Legale: Guidiamo le donne nel complesso sistema legale, spiegando i loro diritti e come intraprendere azioni legali per proteggersi.
  • Orientamento ai Servizi del Territorio: Aiutiamo a connettersi con i servizi del territorio, garantendo l’accesso a risorse come l’assistenza sociale, la protezione e le opportunità lavorative.
  • Percorsi di Autonomia: Accompagniamo ogni donna nel cammino verso l’indipendenza, supportando il reinserimento lavorativo e favorendo la costruzione di una nuova vita, libera dalla violenza.
Impegno per la Prevenzione e la Sensibilizzazione

Crediamo fermamente che la violenza di genere debba essere prevenuta, oltre che affrontata. Per questo, siamo impegnati in progetti educativi e campagne di sensibilizzazione, con l’obiettivo di promuovere una cultura del rispetto e della parità. Tra le attività principali ci sono:

  • Progetti Educativi nelle Scuole: Sensibilizzazione dei giovani al rispetto reciproco e prevenzione dei comportamenti violenti.
  • Campagne di Sensibilizzazione: Attività informative per far conoscere i segnali della violenza e le risorse a disposizione di chi ne è vittima.
  • Formazione per Operatori e Istituzioni: Formazione per migliorare la rete di supporto e garantire una risposta tempestiva ed efficace.

  • Listening and psychological support: We provide a welcoming, non-judgemental space, offering psychological support to help address the trauma of discrimination and to restore psychological and emotional wellbeing.
  • Legal advice: We help LGBTQIA+ people understand their legal rights and take legal action in cases of discrimination, abuse or violence. We support individuals throughout the process to protect their dignity and safety.
  • Signposting to local services: We guide people towards available local social, health and psychological support services, ensuring access to resources that promote social inclusion and wellbeing.
  • Inclusion and independence pathways: We work to ensure that everyone can access employment, educational and social opportunities without being hindered by discrimination, supporting the development of an independent life free from prejudice.
Commitment to prevention and awareness-raising

The fight against discrimination linked to sexual orientation and gender identity is not limited to the protection of rights; it also includes an active commitment to prevention. For this reason, we are involved in:

  • Educational projects in schools: We promote a culture of respect, inclusion and acceptance of diversity, working with younger generations in order to prevent discriminatory attitudes and behaviours.
  • Awareness-raising campaigns: Through information activities and public campaigns, we work to raise awareness of the rights of LGBTQIA+ people and the resources available to those who experience discrimination.

In our Centri Diurni (day centres), dedicated in particular to people with Alzheimer and related dementias, we are committed to stimulating interests and nurturing our residents’ talents, in order to promote the maintenance of residual cognitive abilities and social skills, with a view to achieving full bio-psycho-social wellbeing:

Our daily commitment

We manage, throughout the national territory, first-line assistance services for applicants for international protection (CPSI, Centri Accoglienza e Trattenimento, Centri di Accoglienza Straordinaria), as well as more than 40 Accoglienza e Integrazione (Reception and integration) projects (S.A.I. network) in collaboration with Local Authorities acting as project lead bodies of the project. Some are dedicated to more vulnerable people, such as unaccompanied foreign minors, women and single-parent households.

We work in partnership with public, private and religious organisations to develop integrated reception projects.

We promote integration through multidisciplinary interventions that contribute to the delivery of individual pathways towards personal development and independence, to build a welcoming and participatory community:

In an increasingly individualistic world, characterised by the crisis of the very concept of community and the emergence of the “liquid society”, the challenge is complex: tackling the risk of societies that are highly fragmented and lacking in solidarity is one of our main objectives.

Reducing social marginalisation requires coordinated action at individual, community and institutional level. We recognise that true value must be rediscovered through the development and implementation of initiatives that support a collectivist society, placing emphasis on values that prioritise group wellbeing, social relationships and community roles. This is why, in our facilities, we work so that migrants and refugees, people with disabilities, people experiencing homelessness, people belonging to the LGBTQIA+ community, women and children can feel like active members of society and develop a renewed awareness of the importance of placing themselves and their life plans at the centre.
